I am not sure on the Jcache or non-default values... i'll look into it and
see if I can find out...

We are not sure what to suspect as it affects random users at random times.

I used to run JInitiator (various versions over the years) on an NT4 TSE
farm - but don't recall any problems similar to that.

Which end do you suspect as causing the issue?

How've you got JInitiator configured? Have you individualised the JCache, or
simply running without it? Are you using any non default values for the JVM?
