There are a multitude of things that can cause repeat HOT failure. What is the B+ running at? I would replace the following components for starters (I see that you have already replaced C4401 and C4402): C4102 C4104 C4108 C4145 CR4118 CR4401 CR4402 Q4302 Then check the following: C4112 C4311 R4114 Also remember you may have a bad flyback.
