[TechAssist] NEC Monitor Model# JC-2002VMA-1(chassis Z38A) Need Flyback Number or Source

I am working on a NEC Monitor Model# JC-2002VMA-1 (Chassis Z38A).
Q560(HPA150) horizontal output is shorted & Flyback seems to have a crack
along the top. The only numbers on the flyback are #05650174 & T01419. Does
any one know where I might find this flyback transformer?
