Very sorry to miss last night’s important meeting. ☹
Nice work on the checklist!
I like that it fits on one page, front/back, for clip board use to travel with 
the drive team (?).
You might label the left column “initials” (for the person performing the 
check—that’s how the real world does it).
You could add a spot on the pre-match check for match number/time/notes/etc.  
Post match could include score details, match comments, etc. to provide a 
methodical tool for use in debriefing each match.
