you ask for it from your phone company, telling them you are canceling the 
contract.

Doubtless they will try and keep hold of your custom by offering you a 
cheaper deal.

Knowing of that you are on t-mobile, I can only say that I know a number of 
people that have left t-mobile because of the generally poor signal coverage 
experienced in many areas.  They can't do much about the quality of the 
network so there is little they can offer.
