I just got done asking my husband about the visuals of slash notation.
He helped me with my music coursework and made sure I could use
Sibelius to make charts and get the notation I needed to happen and
look visually correct.
He says your description of the slash going from the second to the
fourth line on the staff in a northeasterly direction is correct. But
in order for this to happen, the notes have to sit on the midline of
the staff. If you're writing in treble clef, that'd be the B
line--Sibelius would play a B4 if you were to try to get an aural
representation of what you're writing. In the bass clef, the notes
would sit on the D line, and Sibelius would play a D3.
The plug-in moves all the notes you want to apply the slash notehead
to so everything will sit on the midline of the staff. Otherwise you'd
have to manually move the notes yourself. That'd be a PITA if you've
got a long passage of slashes.
You'd apply the chord symbols over their respective notes the same way
it would happen if you were using regular noteheads.

What I am trying to do has nothing to do with pitches, there are just
sections of music in a jazz arrangement where all you want to present in
each measure is one slash in each quarter note beat, with chord symbols
aligned over the appropriate beat, but there are no pitches, the slashes
should go from the second line up in a northeast direction to the fourth
line. I will try the things the two of you have suggested and hopefully one
of them will accomplish what I am trying to do.
If you want to know a bit of music trivia, I was taught in a jazz workshop
that these slashes are called virgules.

There's a Plug-in written by Bob Zawalich to transpose your selected
notes to the midline of the staff and change the noteheads to whatever
you need them to be.
Go to

and hit the 2 on the numbers row till you hear:
Move Pitches to Transposed Mid-Line
Download the plug-in and install it.
Here are the steps for installation:
Download the plugin as a zipped file. Open the zipped folder and copy
the plugin file to the clipboard.
Go to root directory and find the "program files(x86)" folder.
Enter on this and find the "Sibelius software" folder.
open this folder and find the "Sibelius 5" folder.
Find the "plugins" folder and open it. Copy the plugin to the
respective subfolder e.g. the "add a pickup bar" plug-in went in to
"Notes and Rests."
