I don't know about the rest but it is impossible for the femoral head to have worn flat in a round hole. The flattening is probably nothing more than a ligament laying across at that point. I would venture to guess if they would re xray it would look different, I pre limed a bitch and had the same flattening at 18 months. OFA'd good at 2 . Never had a limp front or rear until she died at 11. I'm not a vet but I would tell the people not to panic.
