yeah...some how I screwed up the last two questions of the survey....not sure I 
have the skills to fix it....tomorrow I will close the survey and ask those two

Completed your survey...BUT....regarding the two questions below...there is no 
place for a response???...i.e. you cannot type an answer below each 
question...normally there is either a box or a space for those types of 
questions....revisit your original survey instrument and there is another 
section for responding to questions requiring a "narrative" as opposed to the 
multiple choice box type questions....
