I just spoke with Phyllis this morning and it seems there is a problem
sometimes when you fax things. You need to send a cover letter stating how
many
pages you are sending (they will then know if some don't come through) or call
them to be sure they got the fax. And please save your receipt showing that
you
have sent the fax and on what date in case it is needed.
Another reminder - put your credit card number on a separate piece of paper
not on the nomination form or entry form. That way it will be destroyed and
won't be floating around somewhere.
