One question that I have is about the self evaluation - do we have a list of
questions for them to answer (even to themselves)?
My wife's company asks each of their employees and guests who come on site to
answer these questions, and if they answer "yes" to any one of them, then are
referred to their health center for a more thorough screening?
1. Have you had in-person close contact with a person diagnosed with
coronavirus disease (COVID-19)?
· Yes
2. Have you traveled internationally
in the last 14 days
3. Are you feeling sick today?
4. Do you have a temperature?
5. Do you have any of the following?
· Cough
· Shortness of breath
6. Do you have any of the following (not related to allergies)?
· Headache
· Runny Nose
· Itchy Eyes
· Watery Eyes
· Sneezing
