[sbe104_toledo] Statewide test

WUPW had to manually relay the test as we forgot to set the header
filter for "statewide" in our box.  We ran the alert a little late BUT
it did run and the test helped us find a product code we forgot to set.
This was our first monthly test on the new DASDEC system.
