Search for "indexlayout" and you will see where to add the new item. This is
in addition to the existing structures in navigation.php, also there should
be a picture registered for it in interface-lib.php. You can use this commit
as an example:

Yes, indeed. We've found that after looking at the code. We managed
ourselves to do it in a recent plugin:
