[racktables-users] Help with Nagios Plugin

For what seems like a relatively straightforward process i'm having a very
difficult time actually getting this plugin to work with v0.20.11
racktables. The steps I've taken below are laid out, but the issue i'm
having is that when I click on the Nagios tab of an object the page appears
blank, see attached image. Has anyone had luck configuring this to work on
their site?
Download files
Upload to server containing Racktables instance
unzip files into workspace directory
copy nagios.php to plugins directory and inc directory
modify connection string to reflect login and password of user and also the
monitoring website
I'm running both environments on centos06 and have installed curl-common
and all other dependents that I could find for this plugin. Monitoring
server is sitting on the same subnet as the racktables server so there
shouldn't be a firewall contention.
