Film for portraits, tri-x would always be top of my list. For lighting, well as others have said in the ideal world you work everything around the subject matter in sympathy with the look you are trying to achieve. But a good starting point is generally a softbox just above the camera pointing downwards with a fair bit of spill in the downwards direction and some reflectors to bounce some of that back up underneath. A little to the left or the right if you want to introduce some character. Watch out for shiny noses. And a yellow gel to smooth out the skintones a little. That would be my default setup.
