I don't even know who reads this list anymore, but we really haven't
released our fixes in forever. For instance, looking through the list
archives, I see we agreed in August 2013 to remove the footnote in 15:156.
In February 2015, Simon confirmed to a concerned reader that said footnote
would be removed in our next release. It still hasn't been removed.
Anyway, I went through and checked that none of these below were fixed
already and just not released, so I'm including my comments inline.
