In webpages, I also always used markup editing, even when was sighted, and that's one of the reasons I started looking into WPF since liked the idea that you could handle design using markup in any case.
Issue was on a windows 7 machine jaws seemed to be trying to interpret even the XAML markup in terms of it's underlying UIA, and thus wouldn't let me use the XAML too well, whereas it worked alright on an XP machine, but on the other hand, since it was outputting UIA, testing front-end/output worked better on the windows 7 machine.
Haven't really tried this with any more recent versions of jaws 11, or the current jaws 12 beta, on either operating system though - will put it on my to-do/look into list...<smile>

VS 2008 GUI designer is nowhere to be useful - not even for sighted folks. XAML editor is perfectly accessible with intellisense (although you may find what you produce by hand with WPF is not accessible to you in the first place!)
