[program-l] Unlabeled button in vs code

Hi,

There is an unlabeled button on the global toolbar in the latest updates of 
VSCode.
I've tested this with JAWS 2018 and NVDA and I'm seeing the same behavior.

For reference, it's the button that brings up the menu with such things as 
command pallet, check for updates etc.

It's not a show-stopper, but it would be nice if this were fixed.
