Single-file PHP script offering web interface for mySQL, and a couple of other DB platforms - management/manipulation, etc. - for merely executing/testing SQL commands against a DB, after logging in, look for the SQL Command link on the database's landing page/table listing page:

And, WAMP, for example, seems to include this by default - just try following URL if you are running WAMP:

http://localhost/adminer
