[phpa] Upcoming release 1.2p4

Hi,
Over the last couple of evenings I've been focussing on and reviewing how
phpa handles server crashes in, for phpa's perspective, the worst possible
cases. From testing and simulating such events, I'm aiming to release 1.2p4
that should give further tolerance in these cases. Admittedly this is soon
after 1.2p3, but phpa *is* an active project!
