Sorry, I may have misunderstood you, I am assuming that the variable 
PHORM_referral is one that you are passing from the form, as this would be the 
only reason you would want to do this: 
$PHORM_referral = ph_DECLARE; 

If this is the case, try changing the name of the field on the form to 
something non Phorm-like, say name="referral", and then try 

$PHORM_SUBJECT = ($referral == "") ? "Application (no referrer)" : 
"Application (referral: $referral)";
