In your config file, you should define 
$PHORM_ALERTTO = ""; // Change to your email address 
$PHORM_TO = ""; // Change to your email address 
$PHORM_SUBJECT = ""; / The subject of the email message 
$PHORM_TMPL = ph_GENERIC; // A generic email template 

as well as the PHORM_MYTABLE and PHORM_MYVARS variables that you have defined 
for MySQL. 

I haven't tried this, but I am pretty sure it should work. GOod luck.
