Actually I tried this morning to put this up on an IIS box and encountered the
same error but as a result of being there found out that the issue actually
comes with lines 64 and 65 of the fileattach.php3

Warning: fread(): supplied argument is not a valid stream resource in
D:\Inetpub\wwwroot\smallbyte\test_app\phorm\lib\fileattach.php3 on line 64

Warning: fclose(): supplied argument is not a valid stream resource in
D:\Inetpub\wwwroot\smallbyte\test_app\phorm\lib\fileattach.php3 on line 65

Alan, am hoping this may help with the issue currently being had on apache,
direction would be appreciated by myself as overall there are no other
handlers as powerful as this one.
