error from opatch installing Apr 2013 PSU

I am trying to install the Apr 2013 PSU using opatch auto, and I get this
error while patching the grid home, oracle 11.2.0.3 on linux x86-64.  The
error below is in the log file:

2013-05-29 18:42:14: /bin/su exited with rc=1

2013-05-29 18:42:14: Error encountered in the command /u01/app/
11.2.0.3/grid/bin/qosctl -autogenerate

2013-05-29 18:42:14: Failed to patch QoS users.

The error 'Failed to patch QoS users' is the only message opatch returns to
the screen.  I found a note about QoS in the patch installation
instructions, but it didnt seem to be relevant to this message.  Has anyone
else hit this?  Any ideas?
