cache read corrupted block

Anyone seen this before, and have any idea what causes it?  It gave us a
little heartache Tuesday night. We managed to recover it using a backup ocr,
but this is the 3rd time this has happened.  Nothing but Oracle running, and
noone tried to fdisk a drive.  We have opened an SR, but its not moving very
fast:
