Joe, When you a ps -ef|grep oracm do you see the OCM background processes? You should see about 10 or so processes. The recommended method is to install 9201 OCM then patch to 9204 OCM (Make sure you patch the installer as well with 9204). There is also a bug with the 9204 OCM where you need to do a dd if=/dev/zero of=<path to shared disk>/quorum.dbf bs=4096 count=65, if you don't do this the oracm will die. If this doesn't work let me know.
