I think step one would be to validate the backup set on the source host. If
clean then check the following Oracle support notes to see if any are close.
There seems to be an issue with small compressed backup set pieces.

just want to share the issue during rman active duplicate for standby
(both pull and push) in
Oracle 12 Oct2016 PSU Exadata x5 .
First I hit bug solved via DISABLE_OOB=on in sqlnet.ora .
