Re Plaq tox imaging I prefer a dense En Face OCT structural slab right
at the photoreceptor layer to render damage - this coupled with high def
oversampled slices and FAF. I think I parked a case on my LinkedIn page a

We also use split it the same one and use one vial for 2 doses. I believe
it is only good for 6 hours after reconstitution though. It's rare that we
have a 2nd patient needing it that same day, though we do save it. Correct
me if I am wrong about the time limit.

We continue to do ICG Angiograms.
I take a 25 mg vial, reconstitute with 8.0 cc sterile water.
My TD is a 2 cc bolus, equal to 6.25 mg per patient.
We get 4 patients per vial.
I refrigerate any excess, and then use next day, only keep for 24 hours,
Max 36 hours.
It’s very economical at $506/ each or 24 pts per box.

When I was at Columbia we would use 50mg in 5cc of sterile water.
I know they have reduced the amount to 25mg per vial since then and also
increased the cost of the dye.
My question is around our responsibilities to follow CDC safety guidelines
and the billing.
The vials are or at least were labeled "Single Use". Following the CDC
guidelines this should be used on one patient and the excess discarded.
As for the billing the amount for patient 2,3,4.... this creates a problem
as part of the reimbursement is to cover the cost of the dye.
Billing Medicare patients 2+ the same as patient #1 would be against
Medicare billing guidelines we were instructed.

Hospital setting would have kittens going off label and refrigerating dose
to keep it. Private practices only can get away with that.
I was at the RHEM meeting last week. Steve Charles, MD, Mr. Retina,
publically stated that he hardly ever orders FA anymore and that "not
necessary to have an imager do it". Thanks Steve.
WHEN we do ICG, we do a combo FA/ICG using the 2cc added to the 2cc FA and
Spectralis. Works great.
You cannot bill separately for the contrast media under any circumstance;
it is bundled into the procedure itself. Sutter tried their darndest to
get around that rule without success. Nice to have on site, on payroll
billing and coding experts at your disposal, thus I can speak with some
authority here. Believe me, if you could do it, Sutter would find a way.
4 ICG patients within a 36 hour period would be stretching it
statistically; the applications are not % found that widely in the human
condition. Must be a VERY busy retinal practice.

At ICOP Singapore we heard from several retina specialists of note that
ICG is THE best way to correctly diagnose Polypoidal Degeneration. Several
large clinical trials for Polypoidal utilize ICG as an essential component.
So in The Asia Pacific region where there are the largest populations
susceptible to Polypoidal disease, ICG is still a significant imaging
modality. I would say the Uveitis specialists at Moran Eye Center use it
more frequently than others have reported on this thread
