Hi Chris
Thanks for your thorough search if ICG/Metformin related articles. I’d like to
point out in support if the JOP letter to the Editor that this letter was
co-authored by a PhD in Pharmacy who was at the time working at the Poison
Control Center at the University of Utah. He was extremely well qualified to
address the ICG/Metformin issue and his explanations of why radio contrast dyes
are a danger to renal health AND his explanation of why ICG is NOT a Fabio
contrast eye were effective and compelling. Someone in the nursing service is
not understanding what they are reading. Too bad. Remember when Duke-Elder said
fluorescein was a vegetable based dye?
Respectfully,
Paula Morris

I am tired of arguing with our hospital nursing and their hierarchy but I wish
someone would deal with all the questionable information. It seems like
nursing looks for every possible reason to restrict even if there is not good
scientific info. Now we insist on reduced dosage of FA dye for renal impaired
patients as well. It now takes longer for the RN to do an interview for FA or
ICG than it does to do the procedure.
As for ICG we make our patients have their GFR provided and proceed if above
60. From 30-60 we have them hold of Metformin for 48 hours and below 30 they
need a Endocrinologist to OK. The problem with this is the Metformin issue is
based on Radiology contrast which is nothing like FA ICG . See the article
written in an old 1998 OPS Journal above. Is ICG choroidal angiography safe
for diabetic patients taking metformin?
We restrict ICG if they are allergic to shellfish or iodine. It is highly
questionable if it is truly possible to be allergic to iodine. See the opinion
of an Endocrinologist that was published for Pulsion Medical form Germany
There is no allergy against indocyanine green dye based on iodine!
Good luck

I remember having this discussion a very long time ago at IU... I think it
depended on their renal impairment level.

Does anyone NOT do ICGs on patients taking Metformin?
Thanks,
