Hello,
I have a question regarding extended IV antibiotics, specifically for our IV
drug users who are admitted with severe cellulitis, endocarditis, or bacteremia
and require IV antibiotic therapy. How do you all handle patients who are
stable enough for discharge but still require IV antibiotics for an extended
period of time and cannot be converted to PO? We do not discharge them, we
keep them in the hospital until their IV course is done, because we do not want
to release them with an accessible IV line to home care our outpatient
infusions. This practice is killing our length of stay and we are not getting
paid for these days because they meet all other discharge requirements.
I am just curious how others in the network handle this.
