We have a pharmacist in the ED on 2 shifts out of 3 24-7 (no unit based night 
coverage…that is done from central pharm), but do have certified pharm techs 
24-7 performing the “Collect Home Med info” function of the process for 
accuracy and completeness that a clinician will validate.

Good Morning All,

Currently our Pharmacy Department is engaged in Medication Reconciliation in 
the Emergency Department for 12 hours per day and this is well received by all. 
 Now in an effort to maximize the skills of our pharmacists we are seriously 
considering adding a technician to the mix. Does anyone use such a model ? 
Anyone willing to share lessons learned and success stories?

Currently some of our staff pharmacists and technicians are expressing concerns 
and anxiety of this change. Any materials you are will to share is appreciated.
