I have a question regarding the preparation and administration of IV phenytoin.
Currently our approved standard of practice is to administer phenytoin
undiluted via a syringe as a slow IV infusion at a rate not exceed 50mg/min.
The problem is our new B.Braun IV pumps do not accommodate syringes, so now I
am looking into the practice of diluting it into normal saline and using a
filter when administering.
What is everyone else out there doing?
Because it can crystalize quickly do you mix it the pharmacy or is nursing
mixing it on the units just prior to infusion?
Joint Commission frowns upon any IV mixing on the nursing unit that is not
considered an emergency so I need to be compliant with their regs too.
My staff and I appreciate any information anyone has to share out there.
