We allow overrides for STAT and Emergent situations and have a Stat and
emergent list for each floor. The Stat and Emergent list was approved by P&T
and is reviewed regularly ( at least yearly). Also, an override is allowed
when Pharmacy is closed but only the Supervisor has that authority ( we use a
user template in Pyxis) this also requires a second licensed practioner to
review the new medication order.
We review the Pyxis overrides on a weekly basis. We chose to review controlled
substances:
· A Pyxis generated report will be sent via email on a weekly basis for
nursing to review overridden medications taken from their unit Pyxis. Reports
will be reviewed for the following:
- Overrides for ALL Controlled medications
- All orders MUST have a valid order and any waste must be documented
with a witness.

As per TJC, as of January 2018, pharmacy needs to have a policy addressing
pharmacy review of medication overrides. How often are other hospitals are
reviewing medications overrides? Is it monthly, quarterly, etc? Are any
