If you're talking about an actually fill in form for a web page...don't
use Word! Not only is it terrible with web pages...the forms in Word are
not forms that will work as you think for a web page. You need an HTML
editor.
Here's a TechTrax article Linda wrote explaining how to use FrontPage to
create a web form. If you have FP, I'd suggest you use that.
