Which version of Outlook? This was a known issue with Outlook 2000 and (I think) 2002...it used to happen to me all the time...the only version where this doesn't seem to happen to me is 2003. I don't think it was as bad with 2002, but I do remember when I ran 2000 that I had to "end task" on Outlook using ctrl+alt+del every time I closed it, or I too would have multiple copies running when I opened it again.
