You will need to use a pdf generating program, such as pdf995 to do this,
then you just print to the pdf printer driver.  When you specify the name
of an existing file it asks if you want to append.  This program is not
free (unless you are willing to put up with adverts being printed in your
document) but the license is IIRC $19.95.
If the PDF you have is copy protected you will need the password fro it, I
think.
