[macvoiceover] VO going back to default settings

Hi all,
I'm encountering a strange behaviour where VO goes back to its default settings
on its own. I have verbosity set to "medium", keyboard commander turned on,
have VO set to "Do nothing" when deleting text, but every once in a while the
settings go back to the verbosity being set to "High", keyboard commander off,
and the pitch to change when deleting text, and I don't know why. Is there a
keystroke I may be pressing accidentally that sets VO back to its default
settings?
