highlight the item and ten press command-shift-t. Example: go to the applications folder and find the app you want to add to the doc. Press Control-Option-f6 to hear it's name and be sure it's selected. Press Command-Shift-t and hear "add to doc". There it is. YOu can do this with documents as well.
