Oh, I think I found it. when it says my name as current user , one item
selected, I needed to interact to choose “log in options” there. Not very
obvious.

No no. Interact with the list of users and navigate all the way to the bottom
to where it says, Login Options, and stop interacting. Then navigate all the
way to the right to the checkbox I mentioned. You probably will have to unlock
the preferences before you can check the box.

If it's the standard login scree, you have to turn it on in User and
Groups preferences under the Login Options area. You will have to unlock the
preferences first, but then simply check the box to use VoiceOver in the
login window.
If you are speaking of the Filevault login screen, this cannot
automatically be turned on.
