It’s not an FCC form.  If you are required to keep a public file, you keep the sales orders in that file with sufficient information to indicate the candidate name, political party, election committee contact (who paid for the ads) and the rate charged by the station.
The National Association of Broadcasters (NAB) has a form that helps stations keep the proper information.  They have all of the information online, including how to calculate the “Lowest Unit Charge” rate and which candidates and organizations qualify.
