If "Mr." is a respectful appellation in the speaker's language, how can it be disparaging in the language of a receptor who has no cognate?

Suppose someone calls me Bullhouse Yost (Bh. Yost) and I understand that to be the rough equivalent of "Mr." ... where's the insult?

Similarly, your majesty, upon realizing "your majesty" is playful and friendly, how could you receive it as derogation? It's all motives, nicht wahr?
