You could use MS Forms for this as it seems to tick all the boxes of your
requirements.

Alternatively you could create a custom list in Sharepoint online and surface
that through a web part or a link to the ‘new item’ form.

You have complete control using either of these solutions and can quickly
export results to CSV or visualise in Power Bi if you are licensed for it.

Hi Everyone.

We have had a request from our business for a solution to allow electronic
ballots for the EB process. We are going to vote and due to Covid we are
needing to look for something that will allow this to be done online.

The key items we need to ensure it can do is

  *   Employees can only vote once
  *   Voting is anonymous
  *   We can manage / extract and analyse results ourselves

Has anyone already done the leg work on finding a solution that would do this?
