All,

I am working with the Supervising Physicians to edit the Standing Orders.  In 
addition to the Stroke Alert change discussed at the last KCAAC, they will 
include the scope of practice for the Oregon Advanced EMT, or AEMT.  This is 
the new level of responder that the State of Oregon has authorized as of last 
January.  Oregon EMT levels are soon going to come in line with the National 
Registry of EMTs (NREMT) and this update will reflect the new names of the EMT 
levels.  Until this change has completely taken effect at the State, the 
equivalencies below will be used in the Standing Orders:

Emergency Medical Responder (EMR)

EMT-Basic (EMT-B)

Emergency Medical Technician (EMT)

Advanced Emergency Medical Technician (AEMT)

EMT-Intermediate (EMT-I)

EMT-Intermediate (EMT-I) (Oregon-specific)

EMT-Paramedic (EMT-P)

Paramedic


KCC will be teaching an AEMT class this coming January and the new Standing 
Orders document should be ready for distribution at the December Ambulance 
Advisory Committee, with an implementation of January.  More on this later, but 
this will stay just ahead of this class needs for local protocols study 
material.

The following is a description of what was done to make up the AEMT scope:


  1.  All the medications and procedures were extended from the EMT (formally 
EMT Basic) into the AEMT,
  2.  The following medications and procedures were added to the AEMT orders 
from the EMT-I scope:
     *   Adult and pediatric IV
     *   Pediatric only IO
     *   Fluid infusion and fluid challenges
     *   Nitroglycerin administration for chest pains only
     *   Narcan
     *   IV Glucagon for hypoglycemia
     *   Adult and pediatric IV Dextrose
     *   IO Dextrose in Pediatric patient
     *   Updraft breathing treatments of Albuterol and Ipratropium Bromide

The total scope of the Standing Orders document and existing scope of practices 
will remain the same.  There was some formatting changes that allow the EMT 
levels to easily determine their scope.  I am sorry about any inconvenience, 
but changes occurred on almost every page, so I will not be creating insert 
pages for your existing documents, a new copy of the entire document will need 
to be made for each agency.
