This is a worm/virus, and likely did not come from her [as you say she
has virus scanned her HD and it is clean to the latest signature]. Any
computer with her address on it from various source files could have
created the worm. The reason virus scans often "seem" clean with this
kind is that the zip file is password protected--meaning some human has
to enter the password to open the file. Virus scanners cannot do this. On
the other hand, if you save the zip file, open it with the password, then
scan the contents, I warrant you will detect the worm/virus.
