  Once upon a time someone in this list asked about an accessible survey site. 
Am I correct? Well, I found one. Qualtris. I needed to take a survey about the 
kind of work I do with my screen reader (state survey) and the survey site they 
used was qualtris. When I open the site the first thing that appeared was a 
link that said: Using a screen reader? Press here to load an optimized site for 
jaws 11 or higher. I did and presto, an accessible servay. The easiest servay 
site I have ever used.
