Accessible Word Forms

Hi All:

Does anyone know of any good on-line resources I can look at to learn
how to produce forms in MS Word which are accessible to JAWS users?  I
know there are tricks to doing accessible word forms, I just do not know
where to look for information on them.  What I mean by accessible word
forms are forms where users have to check a box, fill in information,
ETC.  I want to learn how to make the check boxes, edit boxes and other
form fields such that JFW users can tell when they are at one that they
may need to check or fill in.  Even my own agency has inaccessible forms
used for various thing; I'd like to change that.
