Q1 - how frequently does this happen - @day, @three days, etc.?
Q2 - what plug-ins are you running?
Q3 - have you examined the server event logs for warnings and errors? 
Q4 - what does the 20 IP addresses mean in this context; does changing
the quantity change the behavior?

I am publishing an internal webserver through ISA. Everything seems to
work fine for a while then the publishing rule seems to stop working.
I start and stop the web proxy and the firewall services and it does not
start working again.
If I restart the ISA server everything works fine again for a few days.
I am publishing a student web-based mailserver and OWA and they never
hang up. I am also publishing terminal services for desktop access for
approx.
10 workstations and they never fail.
It's difficult to tell that it isn't working unless I try to access it
from the outside.There doesn't seem to be any errors in the logs when
this happens. I have approx 20 IP adresses that we are using on the
external nic.
Any Help would be appreciated.
