Yep.  I have all that. Does the machine using VNC have to be a secure NAT
client? Currently it is just a friewall client.

It should.  I have it working both ways to one system.

After you create the protocol definition for port 5900 and call it xxx, make
sure your protocol rule select this new xxx, and apply it to the right
client address set (your internal system, any request...)

Can anyone who has it working let me know what ports/protocols you
created/allowed for VNC to work through the ISA server outbound? In other
words, I want to be able to VNC into a computer on the internet from my LAN
through the ISA server. I had thought port 5900 TCP was all that was needed
but that does not seem to be doing it.

I created a protocol definition for the port and a rule allowing the
protocol to pass.
