ISA does log all that information.
If you mean how to aggregate all the log entries into a report of that
format, then you're talking about writing a log parsing tool.
ISA reports (as you've noticed) aren't configurable.
WebTrends has a firewall log analyzer that might just do what you need.

Dear All,

Thanks for your usual assistance so far. Can anyone
kindly give me a step by step procedure to log all web
sites visited by each user per day. This will enable
me to monitor if that user has violated the company
policy on sites to visit.

I've only been able to log all sites visited per day
by all users, but i want for every user.
