Packet filters and IP-restrictions in your server publishing rules are the 
simplest way for ISA to
handle this.
You may also want to look at locking down you SMTP server.

It appears my system has been Hijacked by Spammers.  How can I block all
IPs from entering through ISA and then gradually allow selected IP and/or
domains access.
