Couple of Questions:
1.  What are the clients?  Windows / Internet Explorer / Netscape? 
2.  What type of authentication?  Basic, Digest, etc?
3.  Is the ISA server a member of a domain or are you using local security?

You may want to check the Web Proxy Log.  If you can see User ID's in the log, 
then the problem will be related to Site and Protocol Rules.  In which case you 
can modify the log file fields to add the rules to do trouble shooting. 

If you don't see user ID's or see them without the proper domain information, 
you have an issue getting the credentials.
